Grades K – 6 (Elementary)

New Students desiring to enroll in the Anchorage Christian Kindergarten or Elementary Schools are required to have the following forms completed:

Elementary Enrollment Form
Statement of Cooperation
Medical Information
Elementary General Policy (Grades 4-6 Only)
Records Request
Leader Recommendation

Elementary Extended Care Enrollment Form

The Leader Recommendation Form should be given to someone in a leadership position who knows your child. Once completed, the form should be sent to the attention of the Elementary Principal at Anchorage Christian Schools.

All other forms should be returned to the Elementary Office along with the necessary fees.

New student enrollment forms must be accompanied by a copy of current shot records and a copy of the child’s birth certificate (for Kindergarten students only). A current physical will be required before school starts.
