Returning 1st - 6th Grade Students

Returning students enrolling in 1st – 6th Grade are required to have the following forms completed:

Elementary Re-Enrollment Form (multiple students may be listed)
Medical Information (one per student)

Additional services are available upon request. Please download the following forms as needed:

All forms should be returned to the Anchorage Christian Schools Finance Office along with the necessary fees.
