Returning Kindergarten Students

Returning students enrolling in Anchorage Christian Kindergarten are required to have the following forms completed:

Kindergarten Re-Enrollment Form
Statement of Cooperation
Medical Information
Leader Recommendation

Additional services are available upon request. Please download the following forms as needed:

Elementary Extended Care Enrollment Form

The Leader Recommendation Form should be given to someone in a leadership position who knows your child. Once completed, the form should be sent to the attention of the Elementary Principal at Anchorage Christian Schools.

All other forms should be returned to the Elementary Office along with the necessary fees.

Kindergarten enrollment forms must be accompanied by a copy of current shot records and a copy of the child’s birth certificate. A current physical will be required before school starts.
